Immediate Loading in Implantology

Immediate loading is the technique that allows a fixed prosthesis to be applied on the implants within 24-48 hours of the surgical procedure. Thanks to the bicortical anchorage, the Bilateral Sinus Solution™ (BSS) can guarantee immediate loading in every case.

What it is

Immediate loading is an implant protocol in which the temporary prosthesis is applied within 24-48 hours of implant placement. Possible only with sufficient primary stability, it contrasts with delayed loading which requires 3-6 months of waiting for osseointegration.

Immediate loading is an implant protocol in which the temporary prosthesis is applied on the implants within 24-48 hours of their placement. This is only possible when the implants achieve sufficient primary stability to withstand the masticatory forces right away.

It contrasts with delayed loading (or conventional), where the implants are left submerged for 3-6 months to allow osseointegration before being loaded. Immediate loading offers the advantage of never leaving the patient without teeth.

Requirements

Traditional immediate loading requires insertion torque greater than 50 Ncm, D1-D3 bone quality, implants of at least 10 mm, at least 4 implants per arch and splinting. Without these conditions traditional implantology requires 3-6 months of waiting.

Traditional immediate loading requires:

If these conditions are not met, traditional implantology cannot guarantee immediate loading and requires waiting times of 3-6 months.

Advantages

The advantages of immediate loading include fixed teeth in 24-48 hours, immediate chewing, immediately restored aesthetics and phonetics, positive psychological benefit, tissue maintenance and a single surgical procedure without the need for a second intervention.

Risks

The risks of immediate loading include implant micro-movements if primary stability is insufficient, implant failure due to failed osseointegration, fracture of the temporary prosthesis and occlusal overload. For this reason traditional implantology carefully selects the cases.

If the requirements are not respected, immediate loading involves significant risks:

For this reason, traditional implantology carefully selects the cases suitable for immediate loading, excluding those with poor or D4 quality bone.

BSS and immediate loading

BSS overcomes the limits of traditional immediate loading thanks to the bicortical anchorage of the pterygoid implants. The torque is always above 50 Ncm regardless of the quality of the alveolar bone, making immediate loading always possible even in cases of severe atrophy.

The Bilateral Sinus Solution™ (BSS) overcomes the limits of traditional immediate loading thanks to the bicortical anchorage of the pterygoid implants.

While in traditional implantology immediate loading is only possible with good quality bone and sufficient volume, in BSS:

This is one of the reasons why the Bilateral Sinus Solution™ (BSS) is considered the most predictable technique for immediate loading even in the most complex cases.
