The Circolare d'Autore® is the definitive prosthesis in zirconia-ceramic made to measure for patients treated with the Bilateral Sinus Solution™ (BSS) technique. With 14 teeth per arch, without fake gum, it represents the maximum in terms of aesthetics, functionality and durability. It is a registered trademark of the Nicola Lettera Maxillofacial Team.

1. What the Circolare d'Autore® is

The Circolare d'Autore® is the definitive fixed prosthesis in zirconia-ceramic that completes the Bilateral Sinus Solution™ (BSS) rehabilitation, applied about 6 months after the procedure. It covers the whole arch with 14 teeth, without fake gum, with a natural appearance. Every prosthesis is a unique handmade piece made to measure for the patient and does not require periodic replacement.

The Circolare d'Autore® (registered trademark) is the definitive fixed prosthesis designed to complete the implant rehabilitation with the Bilateral Sinus Solution™ (BSS) technique. It is applied about 6 months after the surgical procedure, when the osseointegration of the implants is complete and the gums have stabilised around the temporary prosthesis.

It is called "Circolare" because it covers the whole circumference of the dental arch (from molar to molar), and "d'Autore" because every prosthesis is a unique handmade piece, made to measure for the patient by a specialised dental technician under the supervision of the Nicola Lettera Team.

2. Explanatory video

The explanatory video of the Nicola Lettera Maxillofacial Team illustrates the characteristics and advantages of the Circolare d'Autore®. It shows how the definitive zirconia-ceramic prosthesis differs from traditional composite resin prostheses, explaining why the absence of fake gum and the zirconia structure guarantee superior aesthetic results and permanent durability.

3. Materials: zirconia and ceramic

The Circolare d'Autore® uses zirconia (ZrO2) for the load-bearing substructure, with mechanical strength up to 1,200 MPa, biocompatibility and lightness. The aesthetic layered ceramic coating is applied by hand by the dental technician layer after layer with firings above 800°C, faithfully reproducing colour, translucency and morphology of natural teeth.

The Circolare d'Autore® uses the most advanced materials available in prosthetic dentistry:

Zirconia (substructure)

Zirconia (ZrO2) is a high-strength ceramic material used as the load-bearing skeleton of the prosthesis. It is milled from sintered blocks with CAD/CAM technology. Properties:

Ceramic (aesthetic coating)

The layered ceramic is applied by hand by the dental technician layer after layer, firing each layer in an oven above 800°C. This handmade process allows:

Why zirconia-ceramic and not composite resin? The composite resin (All-on-4/6, Toronto Bridge) wears out in 4-5 years, stains, scratches and accumulates bacteria. Zirconia and ceramic are permanent, do not wear out, do not stain, have superior aesthetics and do not need replacement.

4. How it is made

The realisation of the Circolare d'Autore® combines digital technology and craftsmanship: digital impressions with an intraoral scanner, CAD design of the zirconia structure, 5-axis CNC milling with micrometric precision, handmade ceramic layering in several firings, mouth trial and finishing, and definitive application with screwing onto the implants.

The realisation of the Circolare d'Autore® is a process that combines digital technology and dental craftsmanship. Here are the phases:

1

Digital impressions (intraoral scan)

6 months after the procedure, a 3D scan of the patient's mouth is performed with an intraoral scanner. No traditional impressions with pastes and trays.

2

CAD design

The technician designs the zirconia structure on the computer, defining shape, thickness and morphology of every tooth, respecting the patient's occlusion and aesthetics.

3

Milling of the zirconia structure

The zirconia structure is milled from a solid block with 5-axis CNC machines. Micrometric precision, tolerances below 20 microns.

4

Ceramic layering

The dental technician applies the aesthetic ceramic by hand layer after layer (dentine, enamel, background colours), firing each layer in an oven. Every prosthesis requires several layering and firing sessions.

5

Mouth trial and finishing

The prosthesis is tried in the patient's mouth to verify shape, colour, occlusion and comfort. Any small adjustments are made in the laboratory.

6

Definitive application

The Circolare d'Autore® is screwed onto the implants with titanium screws. The prosthesis is definitive: it never needs to be removed except for periodic controls.

5. Differences with other prostheses

The Circolare d'Autore® differs radically from Toronto Bridge and All-on-4/6: it offers 14 zirconia-ceramic teeth without fake gum with permanent durability, while the alternatives have 10-12 composite resin teeth with fake gum and a lifespan of 4-5 years. The Circolare d'Autore® allows normal hygiene, has a very low risk of peri-implantitis and is a one-time investment without recurring costs.

7. Why without fake gum

The fake gum (pink flange) is a typical element of traditional prostheses (Toronto Bridge, All-on-4/6, overdenture) that causes hygiene, aesthetic and health problems. The Circolare d'Autore® completely eliminates the fake gum thanks to the posterior anchorage: the teeth emerge from the natural gum like real teeth, facilitating hygiene and preventing peri-implantitis.

8. Hygiene and maintenance

The maintenance of the Circolare d'Autore® is simple, as with natural teeth: brushing 2-3 times a day, dental floss, interdental brushes and semi-annual check-ups. It does not require specific products or particular techniques. The fixing screws are checked periodically and the prosthesis never needs replacement.

9. From Toronto Bridge to Circolare d'Autore®

The transition from Toronto Bridge to Circolare d'Autore® represents an epochal quality leap: the Toronto Bridge is a composite resin prosthesis with fake gum lasting 4-5 years; the Circolare d'Autore® is in zirconia-ceramic without fake gum with permanent duration. The difference is comparable to that between a sheet metal body and a titanium one.
