How the Bilateral Sinus Solution™ (BSS) solves all the failures
Any patient who entrusts himself to a commercial or traditional dental centre inevitably clashes with a series of standardised, invasive protocols often destined for painful compromises. Below you will find the complete index of the failures and hidden limits of the old dentistry and how the Bilateral Sinus Solution™ (BSS) avoids and solves them.
The case: All-on-4 failure, 20 years of denture — solved in 2 hours
A dear patient of ours comes from an All-on-4 failure, then 20 years of dentures. She discovered Nicola Lettera and his Bilateral Sinus Solution™ technique, called us from Sardinia and told us everything: we told her to come to us and stay calm — you will see that you will solve it. And it went exactly like that: with our technique, in 2 hours we solved everything.
Today, with the Bilateral Sinus Solution™ with grid, she had the immediate loading: immediately she wears the fixed temporary prosthesis (made with 2 posterior basal implants and frontal grid). After 4 months she wears a definitive Circolare d'Autore®: 14 teeth, all molars, without gum.
All-on-4 failure + 20 years of denture: solved in 2 hours with BSS
Immediate loading with 2 posterior basal implants and frontal grid: temporary at 10 teeth. After 4 months the Circolare d'Autore®: 14 teeth, all molars.

The failure of the bone harvesting and grafts
The problem: undergoing double surgical procedures (chin or hip) to harvest bone blocks to screw into a mouth that no longer has support, with months of waiting and zero guarantees of success. The grafts are vulnerable and tend to resorb under the load of daily chewing. The BSS solution: zero harvesting, zero hospitalisations and immediate anchorage in the deep basal bone in just 2 hours.
The problem of the bone grafts and of the autologous bone harvesting is twofold:
- Double surgical procedures (chin or hip) to harvest bone blocks to screw into a mouth that no longer has support
- Months of waiting and zero guarantees of success: the grafts tend to resorb under the load of daily chewing, exposing the patient to infections and long-term failures
The BSS solution: zero harvesting, zero hospitalisations and immediate anchorage in the native and eternal pillars of the basal bone, that do not wear out, completely bypassing the need for artificial bone reconstructions. Learn more →
The failure of the major maxillary sinus lifts
The problem: detaching the Schneiderian membrane and filling the sinus cavities with graft material, risking perforations, chronic sinusitis and months of agony without fixed teeth. The BSS solution: zero sinus lifts and zero risks of membrane laceration, thanks to a targeted surgical path that bypasses the maxillary sinus using the basal bone.
The maxillary sinus lift (sinus lift) involves detaching the Schneiderian membrane and filling the sinus cavities with graft material. The risks are known: perforations, chronic sinusitis and months of agony without fixed teeth.
The BSS solution: zero sinus lifts and zero risks of membrane laceration, thanks to a targeted surgical path that bypasses the maxillary sinus using the basal bone (the same historical principle of the sinus cerclage, evolved in the BSS protocol). Learn more →
The failure of the zygomatic implants
The problem: extra-long and invasive screws (up to 50 mm) that start from the mouth to embed into the zygoma, due to the inability to manage the atrophy with anatomical methods. The BSS solution: no extra-long implants in the zygoma and no demolitive facial surgery, but a primary stability obtained with the native basal pillars.
The zygomatic implants are extra-long screws (up to 50 mm) that cross the facial districts to anchor to the zygoma: invasive surgery, high risks of chronic sinusitis, nerve lesions and complications. Learn more →
The BSS solution: no extra-long implants in the zygoma and no demolitive facial surgery, but a primary stability obtained with the native basal pillars in 18/28 and 15/25.
The limits of the All-on-6 implantology
The problem: 6 implants placed in the resorbing alveolar bone, with posterior limits and fake gums that create infections. The BSS solution: anchorage in the deep basal bone, all molars restored, zero fake gums.
The All-on-6 relies entirely on the alveolar bone: even with 6 implants, if the sinuses are "descended" the posterior implants are precarious and the prosthesis must use bulky fake gums. Learn more →
The limits of the All-on-4 implantology
The problem: 4 implants only in the premaxilla, without posterior molars, with mandatory cantilever and overload of the anterior pins. The BSS solution: posterior pillars in 18/28, complete arch with 14 teeth, zero cantilever.
The All-on-4 places only 4 implants in the anterior area: the arch is incomplete without molars, with cantilever and fake gums. Learn more →
The limits of the Toronto Bridge prosthesis
The problem: a bar with thick fake gum that covers part of the palate, often without molars, a nest of bacteria and peri-implantitis. The BSS solution: the Circolare d'Autore® without fake gum, hygienic, with all the molars.
The Toronto Bridge is the classic fixed prosthesis with fake gum: it creates closed spaces where food stagnates, favouring peri-implantitis, and often leaves the patient without true molars. Learn more →
The failed patient: the case that most needs a network
The patient who has already suffered failures, dentures and negative diagnoses is the case that most needs a network of skills: the Nicola Lettera Network analyses the case from scratch, designs the retreatment with the BSS and rehabilitates the patient with 14 fixed teeth in 2 hours.
