The Prosthesis Replacement Service of the Nicola Lettera Maxillofacial Team is an exclusive offer for patients already wearing fixed prostheses on implants (Toronto Bridge, All-on-4, All-on-6, prosthesis on zygomatic implants) who wish to improve the aesthetics and functionality of their rehabilitation. It allows moving to the Circolare d'Autore® in monolithic zirconia-ceramic, eliminating the annoying pink fake gum.
The Prosthesis Replacement Service is an exclusive offer of the Nicola Lettera Maxillofacial Team aimed at patients already wearing fixed prostheses on implants (Toronto Bridge, All-on-4, All-on-6, prosthesis on zygomatic implants) who wish to improve the aesthetics and functionality of their oral rehabilitation.
Many patients, after years of use, find themselves with worn, unaesthetic prostheses or with pink fake gum that creates embarrassment and hygiene difficulties. The service offers the possibility of moving to the Circolare d'Autore®, the monolithic zirconia-ceramic prosthesis without fake gum.
Evaluation of the existing implants
The preliminary evaluation for the prosthesis replacement includes: CBCT to evaluate the state of the bone and the integration of the existing implants, clinical evaluation of the implant stability and of the state of the soft tissues, occlusal analysis to verify the correct load distribution and aesthetic evaluation of the emergence profile and of the position of the implants.
The first step is an in-depth evaluation of the already present implants:
- CBCT (Cone Beam CT) to evaluate the state of the bone and the integration of the implants
- Clinical evaluation of the implant stability and of the state of the soft tissues
- Occlusal analysis to verify the correct load distribution
- Aesthetic evaluation of the emergence profile and of the position of the implants
Possibilities: keep or replace the implants
Based on the evaluation, two scenarios are possible: maintenance of the existing implants if well osseointegrated and correctly positioned, with realisation of a new Circolare d'Autore® prosthesis adapted to the present abutments (least invasive option); or replacement of the compromised implants (for peri-implantitis, malposition or fracture) with the Bilateral Sinus Solution™ (BSS) technique.
Based on the evaluation, two scenarios are possible:
Maintenance of the existing implants
If the implants are in good condition, well osseointegrated and correctly positioned, it is possible to keep the existing implants and realise a new Circolare d'Autore® prosthesis that adapts to the already present abutments. This option is the least invasive and the fastest.
Replacement of the implants
If the existing implants are compromised (peri-implantitis, malposition, fracture), the Nicola Lettera Team can proceed to the removal of the failed implants and their replacement with new implants positioned according to the Bilateral Sinus Solution™ (BSS) technique, guaranteeing a solid base for the new prosthesis.
Realisation of the new Circolare d'Autore®
The realisation of the new Circolare d'Autore® involves: intraoral digital scan of the arches and implants, CAD/CAM design of the zirconia prosthesis, realisation in monolithic zirconia-ceramic with aesthetic ceramic layering, and trial with mounting in a single session. The result is a custom-made prosthesis, indistinguishable from natural teeth.
