Prevention and Maintenance of the Implants

Daily hygiene
The daily hygiene is essential to prevent peri-implantitis. With the Circolare d'Autore® the cleaning is simplified because there is no fake gum and the zirconia-ceramic surfaces are smooth. A soft-bristle toothbrush, dental floss under the prosthesis, interdental brushes, water flosser and antiseptic mouthwash only for limited periods are recommended.
The daily cleaning of the dental implants with fixed prosthesis is essential to prevent peri-implantitis, the main cause of late failure. With the Circolare d'Autore®, the hygiene is simplified because there is no fake gum and the zirconia-ceramic surfaces are smooth and easy to clean.
Instruments and techniques
- Soft/medium-bristle toothbrush: brush the teeth (implants and prosthesis) at least 2-3 times a day, with particular attention to the junction area between prosthesis and gum
- Fluoride toothpaste: use a normal toothpaste, without excessive abrasives. A fluoride toothpaste is recommended to prevent caries on the residual natural teeth
- Dental floss: pass the floss under the prosthesis and between the prosthetic elements, where the toothbrush does not reach. With fixed prosthesis on implants, the floss must be passed carefully under the bridge
- Interdental brush: use brushes of adequate size to clean the spaces between the prosthetic elements and under the prosthesis
- Water flosser (Waterpik): very useful to remove food residues and plaque in the difficult areas. Set a medium pressure to not irritate the gums
- Antiseptic mouthwash: use a chlorhexidine-based mouthwash (without alcohol) for 1-2 weeks after the procedure, then only on the dentist's indication for limited periods
Periodic controls
A clinical control every 6 months with evaluation of the implant stability and of the occlusion, professional hygiene every 6-12 months with specific instruments for implants such as titanium curettes or Teflon tips, control radiograph every 1-2 years to verify the bone level and occlusal evaluation with night bite in case of bruxism are recommended.
The professional maintenance is a pillar of the implant longevity. We recommend:
- Clinical control every 6 months: maintenance visit with evaluation of the implant stability, of the state of the soft tissues, of the occlusion and of the functionality of the prosthesis
- Professional hygiene every 6-12 months: professional oral hygiene session with specific instruments for implants (titanium or plastic curettes, ultrasonic scalers with Teflon tips) to remove plaque and tartar without scratching the implant surface
- Control radiograph every 1-2 years: intraoral radiograph to verify the peri-implant bone level and identify early any bone loss
- Occlusal evaluation: verification of the correct distribution of the masticatory loads and of the stability of the bite. In case of bruxism, the use of a night bite is recommended
Warning signs
The warning signs include spontaneous or contact gingival bleeding, pain on chewing or digital pressure, mobility of the prosthesis or implant, persistent redness and swelling, pus suppuration, persistent halitosis that does not pass with the hygiene, gingival recession with exposure of the implant and cleaning difficulties.
It is important to know how to recognise early the signs that indicate an implant problem. In the presence of any of these symptoms, contact your dentist immediately:
- Gingival bleeding: spontaneous or contact bleeding (toothbrush, floss) in the implant area
- Pain: spontaneous pain, on chewing or on digital pressure
- Mobility: of the prosthesis or of the implant
- Redness and swelling: persistent
- Suppuration: pus in the implant area
- Halitosis: persistent bad breath that does not pass with the hygiene
- Gingival recession: with exposure of the implant
- Cleaning difficulties: food that gets stuck under the prosthesis
Professionals
Professional maintenance must be performed by trained staff: dental hygienists with experience in implantology and dentists with specific skills in the management of prostheses on implants.
